Support Coordinator Supervisor (SCS)


Position Scope

 

To ensure quality and safe delivery of care

To coordinate provisions of Solomon North HomeCare, LCC services that reflect the philosophy and standards.

To plan, develop, implement, and evaluate Solomon North HomeCare, LLC services, programs, and activities.


Position Qualifications

 
  • One to three years of Supervisory experience

  • A Bachelor’s degree in a human services related field including but not limited to Psychology, Education, Counseling, Rehabilitation Counseling or General Studies with a major concentration in a human services-related field from an accredited institution.

  • A licensed registered nurse

  • A Bachelor’s or Master’s degree in Social Work from a social work program accredited by the Council on Social Work Education.

  • At least 18 years of age, but shall also be at least three years older than all clients under the age of 18

    A minimum of one years of experience working with clients of the population served, be equivalently qualified by education in the human services field, or have a combination of work experience and education with one year of education substituting for one year of experience

  • SCs are supervised to protect clients and staff


Physical Requirements

 
  • Visual/hearing ability sufficient to comprehend written/verbal communication.

  • Ability to perform tasks involving physical activity, which may include heavy lifting, pushing pulling, extensive bending, and standing.

  • Ability to deal effectively stress.


Performance Responsibilities & Standards

 

1. Ensure quality and safe delivery of services.

a. Provide supervision utilizing evidenced-based techniques

b. Serve as resource person for other Support Coordinator professionals

c. Attend and participate in care conferences, treatment planning activities, and discharge planning.

d. Provide oversight and supervision of the Support Coordinators.

e. Function as an advocate for the clients.

f. Ensure the providers adheres to the rules and regulations related CPOC and caseload.

g. Ensure services provided are within the person’s scope of practice.

h. Assist the Administrator/Chief Executive Officer and Governing Body with the development and implementation of policies and procedures.

i. Developed, documented, and implemented Solomon North HomeCare policies and procedures reflect quality and safe delivery of care.

j. Developed and implemented clinical and nursing standards are evaluated on an annual basis.

k. Ensure CPOCs are developed, implemented, and evaluated per policy and procedures.

l. Consistent and concise lines of authority and responsibility are interpreted and maintained among the staff members.

m. Participate in appropriate committee meetings is demonstrated per policy and procedures.

n. Submission of written reports to the Administrator in a timely fashion is demonstrated.

o. Participation in the events of Solomon North HomeCare

p. Positive interpretation of Solomon North HomeCare philosophy, values and long-range plans is provided to the staff members on a regular basis.

q. Consultation with the Administrator is initiated as issues and concerns require.

r. Develops, implements, and coordinates quality management, utilization review, and discharge planning programs.

s. Engages in studies and investigations related to improving delivery of services.

2. Financial/Budget

a. Formulates budget based on appropriate workload indicators and anticipated revenues in conjunction with Administrator.

b. Monitors and controls department costs to maintain expenditures within allocations.

c. Recommends acquisition of capital equipment.

3. Develops, implements, and evaluates adequate staffing needs for delivery of safe and quality of care.

a. Develops, implements, and evaluates methods to ensure effective and efficient use of staff, facilities, equipment, and other resources while maintaining client care standards.

b. Direct or conduct recruitment, hiring and training of clinical personnel.

c. Establish work schedules and assignments for clinical staff, according to workload, space and equipment availability.

d. Developed, implemented, and evaluated the recruitment policies and procedures reflect staffing needs of the home.

e. Solomon North HomeCare recruitment policies and procedures and demonstrated in the interviewing and hiring of qualified staff according to their job written job descriptions.

f. Developed and implemented Solomon North HomeCare staffing patterns that meet the needs of the facility.

g. Staffing patterns assessed to ensure that staffing needs of the facility needs are adequately fulfilled.

h. Implemented staffing patterns are posted in a timely manner.

4. Monitors supervision and evolution of the facility staff job performance

a. Job descriptions are reviewed and revised annually.

b. Direct, supervise and evaluate work activities of clinical personnel.

5. Education/Inservices/Orientation

a. Plans and organizes orientation and in-service training for staff members and participates in guidance and educational programs.

b. Direct or conduct recruitment, hiring and training of clinical personnel.

c. Plans and organizes orientation and in-service training for staff members and participates in guidance and educational programs.

d. Direct, supervise and evaluate work activities of clinical personnel.

6. Computer:

a. Must excel in computer use to include, but not limited to, word, excel and email as well as other forms of communication through technology.
