Support Coordination Services
Support Coordination (formerly case management) is defined as services provided to Medicaid eligible individuals to assist them in gaining access to needed waiver and other State Plan services as well as needed medical, social, educational, and other support services within the Office for Citizens with Developmental Disabilities (OCDD).
SNSCST services clients with the following:
Developmental disability means a severe, chronic disability of an individual that:
attributable to a mental or physical impairment, or a combination of mental and physical impairments, other than a sole diagnosis of mental illness
manifested before the individual reaches 22 years of age; 3.
likely to continue indefinitely
A child from birth to age nine, inclusive, who has a substantial developmental delay or specific congenital or acquired condition may be considered to have a developmental disability
There are many conditions that qualify as a developmental disability including autism, brain injury (before age 22), cerebral palsy, other mental or neurological conditions (seizures) and intellectual disabilities which include Down syndrome, fetal alcohol spectrum disorder (FASD). Other developmental disabilities may be strictly physical, such as blindness or deafness that began from birth or childhood.
